For people on night or rotational shifts, a blood test has two usual obstacles: a morning lab window and an overnight fast. A Thyroid Profile has neither. It reports TT3, TT4 and TSH from one serum sample, needs no fasting, and can be drawn when you come off shift or before one begins. There is one thing worth planning, though, and that is which hour you choose, because TSH follows a daily rhythm. ## Why the Hour of Sampling Matters for TSH TSH is not constant through the day. In people on a conventional sleep pattern it tends to be highest during the night and lower in the afternoon, and cortisol follows its own rhythm too. For a night-shift worker, a 7 AM sample taken straight after work is therefore not the same physiological moment as a 7 AM sample from someone who has just woken. This does not make the test unusable. It means your doctor should know your shift pattern, and that consistency matters more than the absolute hour: if you are tracking treatment, give each sample at about the same point in your own cycle. ## What Each Result Measures Thyroid Stimulating Hormone (TSH) is released by the pituitary and prompts the thyroid to work; it rises first when the thyroid underperforms. Thyroxine - Total (TT4) is the main thyroid hormone. Tri-Iodothyronine Total (TT3) is the more active form, produced mostly by conversion of T4 in tissues. "Total" includes hormone bound to carrier proteins as well as the free fraction. ## Interpreting the Combination High TSH with low TT4 means an underactive thyroid. High TSH with TT4 inside range is subclinical hypothyroidism and is commonly rechecked after some weeks. Low TSH with raised TT4 or TT3 suggests an overactive thyroid. About one in ten Indian adults is hypothyroid, and women are affected about three times as often as men (15.86% versus 5.02%) (Unnikrishnan et al., 2013). Tiredness, weight change and low mood overlap with the effects of broken sleep, so thyroid disease can be missed among shift workers who assume the shift explains everything. ## Practical Notes for Noida Shift Workers A phlebotomist can visit homes in Sector 62, Sector 137, Sector 78, Sector 120 or along the Noida–Greater Noida Expressway at a time set around your sleep. Do not skip rest to attend a lab. If you take levothyroxine, ask your doctor whether to take the dose before or after the draw, and keep that routine the same on every test. ## Limits of This Profile It does not explain the cause of a thyroid problem; autoimmune thyroiditis needs antibody testing such as Anti-TPO. Total T3 and T4 can shift with binding protein changes in pregnancy or with oestrogen-containing medicines. Acute illness and some drugs disturb TSH temporarily, and biotin supplements can interfere with some immunoassays. It does not measure sleep debt or circadian disruption, so a normal result does not mean the tiredness is imagined. Neck lumps need an ultrasound. ## Packages That Include It If you need other panels as well, a package costs less than separate bookings. Full Body Checkup with Thyroid (80 parameters) is the first package containing this profile; Advanced Full Body Checkup with Vitamin D & B12 (89 parameters) and higher packages include it too. All packages contain a Lipid Profile and Fasting Glucose and need a 10-hour fast, which is awkward after a night shift; this profile on its own is not. ## What Is Not Included No Free T3 or Free T4, no Anti-TPO or anti-thyroglobulin antibodies, no thyroglobulin, no reverse T3, no cortisol, no thyroid ultrasound, and no Vitamin D, Vitamin B12 or blood count. Each is a separate order. Processing is at a NABL-accredited laboratory in our network; accreditation is granted to individual laboratories after evaluation of equipment, processes and personnel, and HealthVas does not claim to hold it. ## Sources Unnikrishnan AG, et al.
