Choosing between the two antenatal profiles comes down to two tests. This one is the Antenatal Profile plus the full Thyroid Profile — Tri-Iodothyronine Total (TT3) and Thyroxine Total (TT4) alongside TSH, instead of TSH on its own — plus Insulin-Fasting, which indicates insulin resistance when read against the fasting glucose. Nothing else differs, and if your obstetrician has not asked for either, the Antenatal Profile covers the same ground. What both are built around are the components that genuinely serve pregnancy care: Blood Grouping (A B O) and Rh Type, which decides whether anti-D prophylaxis is needed; CBC with ESR for anaemia, which affects over half of Indian women of reproductive age; a pathologist-read Blood Picture - Peripheral Smear Examination showing which kind of anaemia; Complete Urine Analysis for urinary infection and protein; and Blood Glucose Fasting. 52 parameters across 10 components. The profile also carries three viral load quantification tests, which measure the amount of virus in someone already known to be infected rather than screening for infection. Pregnancy care should follow the schedule your obstetrician sets. From 1499. Free home collection across Delhi.

What Does This Advance Antenatal Profile Cover?

At a Glance

Package: Advance Antenatal Profile. Tests: 52 parameters, 10 components. Price: From 1499. Fasting: 8–10 hours, for the Blood Glucose Fasting and Insulin-Fasting components. Sample: Blood and urine, single visit. Reports: 24 hours, digital. Home collection: Free across Delhi. Lab: NABL-accredited. The word "Advance" does less work here than it looks. This is the Antenatal Profile with two tests added — the full Thyroid Profile in place of TSH alone, and Insulin-Fasting. Every other component is the same. If your obstetrician has not asked for either, the Antenatal Profile covers identical ground. ## The Two Additions, Stated Plainly Thyroid Profile (3): Tri-Iodothyronine Total (TT3), Thyroxine - Total (TT4) and Thyroid Stimulating Hormone (TSH), replacing the single TSH test. Maternal hypothyroidism affects fetal neurodevelopment and thyroid testing is standard in pregnancy; one in ten Indian adults is hypothyroid, women about three times as often as men, 15.86% against 5.02% (Unnikrishnan et al., IJEM, 2013;17(4):647–652). TSH alone catches most cases; TT3 and TT4 show where in the pathway the problem sits. Pregnancy shifts thyroid reference ranges by trimester, and total T3 and T4 are pushed up by rising binding proteins, so these need trimester-specific reading. Insulin-Fasting (1): circulating insulin on a fasting sample. Read against the fasting glucose it indicates insulin resistance, the mechanism behind gestational diabetes, which can be present while glucose still reads normal because the pancreas compensates. The limitation: fasting insulin does not diagnose gestational diabetes. That is the oral glucose tolerance test your obstetrician schedules, usually at 24–28 weeks. ## Your Obstetrician Sets the Schedule Antenatal care in India is organised visit by visit, and that schedule belongs to the obstetrician treating you. This profile supports that care; it does not satisfy or replace an antenatal protocol and is not a first-trimester workup. Two standard elements of Indian antenatal care are missing from it: VDRL or RPR for syphilis and rubella IgG. Antenatal infection screening — HIV 1&2 antibody, HBsAg, anti-HCV — is also not what the three viral tests here do. ## Blood Grouping (A B O) and Rh Type One parameter, with more riding on it than anything else here. An Rh-negative mother carrying an Rh-positive baby can form antibodies against fetal red cells; a first pregnancy is usually unaffected, but in a later one those antibodies cross the placenta and destroy the baby's red cells — haemolytic disease of the newborn. Anti-D immunoglobulin prophylaxis prevents almost all of it, on the obstetrician's schedule, but only once the Rh status is on record. The ABO group is separately required before any transfusion. ## The Three Viral Tests Measure Quantity, Not Presence This profile carries HIV - Quantitative (Viral Load), Hepatitis B Virus - Viral Load and Hepatitis C Virus- Viral Load. All three are viral load quantification assays: they measure how much virus is circulating in someone already known to carry that infection, and exist to monitor disease and antiviral treatment. A viral load result is a monitoring number. It does not answer "do I have this infection?" — antibody and antigen screening tests do that, and your doctor will advise on them. ## Blood Picture - Peripheral Smear Examination Hand work, not machine work: a pathologist prepares a slide and reads it under the microscope cell by cell. That is pathologist time, the first item cut from cheaper pregnancy packages. Five findings — Red Blood Cells, White Blood Cells, Platelets, Hemoparasites, and the pathologist's written Impression. Anaemia is why it belongs here. Over 50% of Indian women aged 15–49 are anaemic (NFHS-5, 2019–21), and the smear says which kind: microcytic pale cells for iron deficiency, macrocytic cells for B12 or folate deficiency, sickle-shaped cells for sickle cell disease, which alters pregnancy management. It also flags blast cells that may point to leukaemia, and platelet clumping that analysers report as a falsely low count — which matters because a true low count changes delivery and anaesthesia decisions. Hemoparasites matters in Delhi NCR, where malaria and dengue are endemic and malaria in pregnancy raises risk for mother and baby. ## Managing the Fast at Home in Delhi This profile needs 8–10 hours without food, and fasting insulin is uninterpretable otherwise. Doing that at home beats a waiting room: the phlebotomist arrives in an early slot, takes the blood and midstream urine, and you eat immediately after — with family at home beside you. Free across Vasant Vihar, Vasant Kunj, Chattarpur, Nehru Place, Jasola, Okhla, Connaught Place, Civil Lines and Model Town. ## The Other 3 Components CBC with ESR (21): haemoglobin, red cell indices, white cell differential, platelets, ESR — the anaemia and infection baseline of pregnancy. Complete Urine Analysis (17): protein, glucose, nitrites, leukocytes, pus cells, casts. Urinary infection in pregnancy is often symptomless, and urine protein is part of pre-eclampsia surveillance; the routine test detects overt proteinuria but not microalbuminuria, which needs a separate urine microalbumin or albumin-creatinine ratio test. Blood Glucose Fasting (1): under 100 mg/dL normal, 100–125 prediabetes, 126 and above diabetes (ADA Standards of Care, 2024). ## Sources NFHS-5, 2019–21 · Unnikrishnan et al., IJEM, 2013;17(4):647–652 · ADA Standards of Care in Diabetes, 2024 Content last reviewed: October 2026

Who Should Consider This Profile?

  • Women with PCOS, a high BMI, or diabetes in the family – Insulin-Fasting is one of only two reasons to pick this profile over the Antenatal Profile. Read against the Blood Glucose Fasting value it indicates insulin resistance — the mechanism underlying gestational diabetes — which can be present before glucose itself rises. India has 101 million adults with diabetes (Anjana, Mohan et al., ICMR-INDIAB, Lancet Diabetes & Endocrinology, 2023;11(7):474-489). Fasting insulin is not diagnostic: that requires the oral glucose tolerance test an obstetrician schedules, usually at 24 to 28 weeks.
  • Women whose obstetrician has asked for more than a TSH value – The other reason to choose this profile. It reports Tri-Iodothyronine Total (TT3) and Thyroxine Total (TT4) alongside Thyroid Stimulating Hormone (TSH), where the Antenatal Profile has TSH alone. Maternal hypothyroidism affects fetal neurodevelopment, and one in ten Indian adults is hypothyroid, women about three times as often as men, 15.86% versus 5.02% (Unnikrishnan et al., Indian Journal of Endocrinology and Metabolism, 2013;17(4):647-652). Thyroid ranges shift by trimester, so interpretation belongs with your obstetrician.
  • Women already on thyroid medication before the pregnancy – Thyroid hormone requirements usually rise in pregnancy, and a woman already taking levothyroxine needs her dose reviewed rather than assumed to be adequate. The full Thyroid Profile here gives TT3, TT4 and TSH from one draw, which is more information for that review than TSH alone. Mention the medication and the time you took it when the phlebotomist arrives, because that affects how the values read.
  • Women whose Rh status is not documented anywhere – Blood Grouping (A B O) and Rh Type carries the single largest consequence in this profile. An Rh-negative mother carrying an Rh-positive baby can form antibodies that, in a later pregnancy, cross the placenta and destroy fetal red cells. Anti-D immunoglobulin prophylaxis prevents nearly all of that, on the schedule the obstetrician sets, but only once the Rh status is on record. The ABO group is separately required before any transfusion at delivery.
  • Women entering pregnancy with anaemia or a history of it – Over 50% of Indian women aged 15 to 49 are anaemic (NFHS-5, 2019-21), and pregnancy raises iron demand sharply. This profile pairs CBC with ESR against a pathologist-read Blood Picture - Peripheral Smear Examination, so the report shows not just a low haemoglobin but what kind of anaemia it is — microcytic for iron deficiency, macrocytic for B12 or folate deficiency, sickle-shaped cells for sickle cell disease. Each needs different treatment.
  • Women who would rather fast at home than in a waiting room – This profile needs 8 to 10 hours without food, because fasting insulin cannot be interpreted on a non-fasting sample. A trained phlebotomist collects the blood and midstream urine at your door across Vasant Vihar, Vasant Kunj, Chattarpur, Nehru Place, Jasola, Okhla, Connaught Place, Civil Lines and Model Town at no extra charge, in the earliest slot available, so you can eat the moment the sample is taken rather than after a journey home.

How to Prepare

Fasting: 8–10 hours overnight. Both Blood Glucose Fasting and Insulin-Fasting need it, and fasting insulin cannot be interpreted at all on a non-fasting sample. Water is allowed and makes the draw easier. No tea, coffee, juice or milk during the fasting window. Managing the fast in pregnancy: A long fast can bring on nausea or light-headedness. Book the earliest morning slot, keep food ready to eat the moment the sample is taken, and have someone at home with you. If your obstetrician has advised against fasting at this stage, say so when booking and ask them what they want instead. Samples: One venous blood draw plus a midstream urine sample in the same visit. For the urine sample, clean the area first, pass the first part of the stream into the toilet and collect the middle portion in the sterile container — contamination is the commonest reason a urine report has to be repeated. Timing: 7–9 AM. TSH follows a daily rhythm and morning readings are the most interpretable, and an early slot keeps the fasting window short. Medication and supplements: Continue iron, folic acid, calcium and prescribed medicines unless your obstetrician says otherwise. Mention iron supplements, which change how the CBC and peripheral smear read, and mention thyroid medication along with the time you took it. Stop biotin 48 hours beforehand — it interferes with immunoassays and can produce falsely normal thyroid results. Recent illness: Mention any fever, infection or antibiotic course in the past two weeks. It affects the white cell count, ESR and urine report. Home collection: Free across Delhi — Vasant Vihar, Vasant Kunj, Chattarpur, Nehru Place, Jasola, Okhla, Connaught Place, Civil Lines, Model Town and Patel Nagar. Reports: Digital within 24 hours, all 52 parameters with reference ranges and flagged abnormal values. Carry the report to your next antenatal visit — the thyroid values in particular are read against your gestational week.

What Is Included — and What Is Not

Included: 52 parameters, 10 components 4 profiles: Complete Blood Count (CBC) + ESR (21), Complete Urine Analysis (CUE) (17), Blood Picture - Peripheral Smear Examination (5), Thyroid Profile with TT3, TT4 and TSH (3). 6 individual tests: Blood Grouping (A B O) and Rh Type (1), Blood Glucose Fasting (1), Insulin-Fasting (1), HIV - Quantitative (Viral Load) (1), Hepatitis B Virus - Viral Load (1), Hepatitis C Virus- Viral Load (1). Sample: Blood and urine, single visit. Fasting: 8–10 hours for the Blood Glucose Fasting and Insulin-Fasting components. Reports: 24 hours. Home collection: Free across Delhi. ## What it adds over the Antenatal Profile Two components, and no more: the full Thyroid Profile (TT3, TT4, TSH) in place of the single Thyroid Stimulating Hormone (TSH) IHC test, and Insulin-Fasting. That is 52 parameters across 10 components against 49 across 9. Nothing else is different. ## What the three viral tests are HIV - Quantitative (Viral Load), Hepatitis B Virus - Viral Load and Hepatitis C Virus- Viral Load are viral load quantification assays. They measure the quantity of virus circulating in a person already known to be infected and exist to monitor disease activity and treatment response. They do not screen for the presence of infection. ## Not included Infection screening is not part of this profile. HIV 1&2 antibody, HBsAg for hepatitis B and anti-HCV for hepatitis C — the tests antenatal protocols use for screening — are not included. Neither is VDRL/RPR for syphilis nor rubella IgG, both standard elements of Indian antenatal care. Also not included: the oral glucose tolerance test (OGTT) that diagnoses gestational diabetes; HbA1c; serum ferritin, vitamin B12 or folate; haemoglobin electrophoresis or HPLC for thalassaemia trait, which is what a microcytic smear calls for next; indirect Coombs and antibody screening for Rh-negative mothers; urine culture and sensitivity; anti-TPO and other thyroid antibodies; free T3 and free T4; the double marker, triple marker, quadruple marker and NIPT; and no imaging of any kind, including the dating, NT and anomaly scans. This profile is baseline laboratory work. It is not a first-trimester workup and does not satisfy an antenatal protocol. Pregnancy care should follow the schedule advised by your treating obstetrician, who decides which tests are needed at which gestational week.

Frequently asked questions

Common questions about Advance Antenatal Profile in Delhi

What is included in the Advance Antenatal Profile?

52 parameters across 10 components: Complete Blood Count (CBC) + ESR (21), Complete Urine Analysis (17), Blood Picture - Peripheral Smear Examination (5), Thyroid Profile with Tri-Iodothyronine Total (TT3), Thyroxine Total (TT4) and Thyroid Stimulating Hormone (TSH) (3), Blood Grouping (A B O) and Rh Type (1), Blood Glucose Fasting (1), Insulin-Fasting (1), HIV - Quantitative (Viral Load) (1), Hepatitis B Virus - Viral Load (1) and Hepatitis C Virus- Viral Load (1). Sample is blood and urine in one visit; 8 to 10 hours of fasting is required.

What exactly does this add over the Antenatal Profile?

Two tests. The full Thyroid Profile — Tri-Iodothyronine Total (TT3) and Thyroxine Total (TT4) alongside TSH — replaces the single Thyroid Stimulating Hormone (TSH) IHC test, and Insulin-Fasting is added. That is the entire difference: 52 parameters across 10 components against 49 across 9. Every other component is identical. If your obstetrician has not specifically asked for a wider thyroid picture or a fasting insulin value, the Antenatal Profile covers the same ground for less.

Do the HIV and hepatitis tests here screen me for those infections?

No. HIV - Quantitative (Viral Load), Hepatitis B Virus - Viral Load and Hepatitis C Virus- Viral Load are viral load quantification assays. They measure how much virus is circulating in the blood of someone already known to be infected, and they are used to monitor disease activity and response to antiviral treatment. Whether an infection is present is answered by different tests — HIV 1&2 antibody, HBsAg for hepatitis B, and anti-HCV for hepatitis C. Ask your obstetrician for those by name if screening is what you need.

Is this profile sufficient as a first-trimester workup?

No. It is baseline laboratory work, not a complete first-trimester workup, and it does not satisfy an antenatal protocol. Antenatal care in India follows a visit-by-visit schedule set by the treating obstetrician, with particular tests at particular gestational weeks. Two standard elements of Indian antenatal care are absent from this profile: VDRL or RPR for syphilis, and rubella IgG. The ultrasound scans, the oral glucose tolerance test and the first-trimester screening markers are not part of it either.

How much does the Advance Antenatal Profile cost in Delhi?

It starts from 1499. That includes free home collection of both the blood and urine sample anywhere in Delhi, and digital reports within 24 hours, with nothing added at checkout. Book online or on WhatsApp.

What does Insulin-Fasting show in pregnancy?

It measures how much insulin is circulating on a fasting sample. Read alongside the Blood Glucose Fasting value, it indicates insulin resistance — the mechanism underlying gestational diabetes — which can be present while glucose is still normal, because the pancreas compensates by producing more insulin. It is most informative if you have polycystic ovary syndrome, a high BMI or a family history of diabetes. It must be taken fasting; a non-fasting insulin value cannot be interpreted at all.

Does a normal fasting insulin mean I will not get gestational diabetes?

No. Fasting insulin is not a diagnostic test for gestational diabetes. The diagnosis comes from an oral glucose tolerance test, in which glucose is measured at intervals after a standard glucose drink, and your obstetrician schedules that separately — usually between 24 and 28 weeks. A normal fasting insulin and a normal fasting glucose early in pregnancy do not rule out gestational diabetes developing later, because insulin resistance rises as the pregnancy progresses.

Is the full Thyroid Profile actually better than TSH alone in pregnancy?

TSH alone detects most thyroid dysfunction and is the usual first-line test, so the honest answer is that it is more information rather than automatically better. Adding Tri-Iodothyronine Total (TT3) and Thyroxine Total (TT4) shows where in the pathway the problem sits. This matters because maternal hypothyroidism is associated with impaired fetal neurodevelopment; one in ten Indian adults is hypothyroid, women roughly three times as often as men, 15.86% versus 5.02% (Unnikrishnan et al., Indian Journal of Endocrinology and Metabolism, 2013;17(4):647-652). Pregnancy shifts thyroid ranges by trimester and total T3 and T4 are raised by binding proteins, so trimester-specific interpretation is essential.

Why does blood group and Rh type matter so much in pregnancy?

Because of Rh incompatibility. If you are Rh-negative and the baby is Rh-positive, your immune system can form antibodies against the baby's red cells. A first pregnancy is usually unaffected, but in a later one those antibodies cross the placenta and destroy fetal red cells, causing haemolytic disease of the newborn. Anti-D immunoglobulin prophylaxis prevents almost all of this, given on a schedule your obstetrician sets — but only when the Rh status is documented. The ABO group is separately required before any transfusion at delivery.

What does the Blood Picture - Peripheral Smear Examination add to the CBC?

Cell shape and structure, which no analyser can see. A qualified pathologist prepares a slide from your blood and reads it cell by cell, reporting five findings: Red Blood Cells, White Blood Cells, Platelets, Hemoparasites, and a written Impression. A CBC says haemoglobin is low; the smear says why — small pale microcytic cells for iron deficiency, large macrocytic cells for B12 or folate deficiency, sickle-shaped cells for sickle cell disease, which substantially changes how a pregnancy is managed. It also detects blast cells that may indicate leukaemia and malarial parasites inside red cells.

Why does the smear matter particularly in a pregnancy profile, and why is it often missing?

Anaemia is the first reason: over 50% of Indian women aged 15 to 49 are already anaemic (NFHS-5, 2019-21), and treatment depends on which type it is, which a cell count cannot tell you. The second is platelets — automated counters misread platelet clumping as a dangerously low count, and in pregnancy that error affects delivery and anaesthesia decisions. The third is Hemoparasites: malaria and dengue are endemic across Delhi NCR, and malaria in pregnancy raises risk for mother and baby. It is often missing because it cannot be automated; a pathologist has to read every slide by hand, so it is the first line item cut to hold a price.

If my haemoglobin is low, does this profile identify the cause?

It points to a cause rather than confirming one. The CBC indices and the peripheral smear together indicate the type — microcytic cells suggest iron deficiency, macrocytic cells suggest B12 or folate deficiency. But this profile does not include serum ferritin, vitamin B12 or folate levels, so those are indications, not confirmations. If microcytic cells are reported, thalassaemia trait is also possible, and that requires a separate haemoglobin electrophoresis or HPLC test. Your obstetrician decides the follow-up.

What does the Complete Urine Analysis detect in pregnancy?

It reports 17 parameters, including nitrites, leukocytes and pus cells for urinary tract infection, plus protein and glucose. Urinary infection is common in pregnancy and often entirely without symptoms, while untreated infection is associated with preterm labour. Urine protein feeds pre-eclampsia surveillance. One limitation: a routine urine test detects overt proteinuria but not microalbuminuria, which needs a separate urine microalbumin or albumin-creatinine ratio test. A result suggesting infection is usually followed by a urine culture, which is not part of this profile.

I live with my in-laws — how do I manage an 8 to 10 hour fast at home?

Easier than most people expect, and easier than managing it at a laboratory. Book the earliest morning slot available so the fast ends sooner, finish dinner by around 9 or 10 PM, drink water freely through the night, and have breakfast ready to eat the moment the sample is taken. Having family at home is an advantage here: someone can keep the food ready and stay with you, which matters because a long fast in pregnancy can bring on nausea or light-headedness. If your obstetrician has advised against fasting at this stage, tell us at booking.

Which Delhi localities can I get an early fasting slot in?

Home collection is free across Delhi, and early morning slots are available throughout — including Vasant Vihar, Vasant Kunj, Chattarpur, Nehru Place, Jasola, Okhla, Connaught Place, Civil Lines, Model Town and Patel Nagar. A trained phlebotomist collects both the blood and the midstream urine sample at your door, and the samples travel to the laboratory in temperature-controlled conditions. Choose the earliest slot offered for your area, since both Blood Glucose Fasting and Insulin-Fasting need the overnight fast.

How fast are reports, and is the laboratory NABL-accredited?

Digital reports arrive within 24 hours of sample collection, with all 52 parameters, reference ranges and flagged abnormal values. Carry the report to your next antenatal visit: thyroid values and haemoglobin are read against your gestational week rather than standard adult ranges, and fasting insulin has to be read alongside the fasting glucose. Your sample is processed at a NABL-accredited laboratory in our network. NABL accreditation is granted to individual laboratories after an evaluation of their equipment, processes and personnel; it is not a blanket brand certification, and HealthVas does not claim to hold accreditation itself.

Can I book this for my wife or daughter-in-law?

Yes. Add her details as the person being tested, and pick the earliest morning slot because of the 8 to 10 hour fasting requirement. Anyone in the household can make the booking and be present when the phlebotomist arrives. One thing to settle with her obstetrician first: whether the full Thyroid Profile and fasting insulin are actually wanted at her current gestational week. If not, the Antenatal Profile covers the same ground, since those two tests are the only difference between the profiles.

Why Book on HealthVas?

The price you see is the price you pay. Home sample collection and digital reports are included, with nothing added at checkout — which matters across a pregnancy involving repeated testing. Every sample is processed at a NABL-accredited laboratory in our network. NABL accreditation (National Accreditation Board for Testing and Calibration Laboratories) is granted to individual laboratories after evaluation of their equipment, processes and personnel — it is not a company-wide certification, and HealthVas does not claim to hold it. We will not dress up two tests as a tier upgrade. The only things this profile adds over the Antenatal Profile are the full Thyroid Profile and Insulin-Fasting. If your obstetrician has not asked for either, book the Antenatal Profile instead and keep the difference. Home collection is the other practical argument: an 8 to 10 hour fast is easier to hold at home than in a waiting room, and you can eat as soon as the sample is taken. Two things we state rather than bury. The Blood Picture - Peripheral Smear Examination is read by a qualified pathologist rather than an analyser, which is how the type of anaemia is actually identified. And the three viral tests here are viral load quantification assays, measuring how much virus is present in someone already known to be infected — they are not infection screening. Antenatal infection screening uses HIV 1&2 antibody, HBsAg and anti-HCV, and VDRL for syphilis and rubella IgG are standard parts of Indian antenatal care that this profile does not include. Free home sample collection across Delhi. Digital reports within 24 hours. Book online or on WhatsApp.
